If a Clinician has requested a sample from you, you are able to collect a pot/swab and the relevant paperwork from our main reception desk, located on the 1st floor of Eagle Bridge Health & Wellbeing Centre.
Any sample must be returned to Reception on a week day (Monday to Friday) and must be before 12:00 noon. Samples are collected and sent for testing after this time, so we can not accept or store any samples that are dropped off after 12:00.
Please make sure your sample is in the correct container, if you are unsure please ask a member of the reception team for help. Your name and date of birth needs to be clearly written on the label and any relevant paperwork must be enclosed.
